In an unmodified stem cell transplant, T cells (a type of white blood cell that plays a vital role in building your new immune system) are not removed from the stem cells until they are transplanted into you. They are given in the same way as for a blood transfusion, over several hours through your tunneled catheter. If you and the donor have different blood types, red blood cells or plasma must be removed before the stem cells are given to you. By killing cancer cells, chemotherapy also destroys hair cells. In most cases, hair falls out during transplantation.
Hair loss is uncomfortable and frustrating for patients, and many choose to cut their hair short before transplantation to control the process themselves and minimize the shock of hair loss. Discuss this issue with your caregiver, friends, and family.

You can order a wig before your hair falls out or bring in your cut hair if you want the wig to match your natural hair color. Ask your doctor for a prescription for a wig, as some insurance companies will partially reimburse you for wigs.
Skin changes
Preparation may cause skin changes such as increased dryness, flaking, discoloration, and darkening. Wash with toilet soap labeled “for sensitive skin.” Talk to your nurse about the use of various remedies. To reduce dryness, use alcohol-free lotions, creams, or oils.
In addition, the skin can be susceptible to sunlight and can quickly burn. Avoid bright sunlight. When you go outside, wear sunscreen with an SPF of at least 30 and wear protective clothing such as long-sleeved shirts, pants, and a hat. The skin is the most common site for cancer, and sunscreen reduces the risk of skin cancer.

Transplant Day
Upon completion of the preparatory therapy regimen, stem cell infusion is performed – a transplantation procedure.
On Day 0, the donor stem cells will be injected into your body. These stem cells will help you make new blood cells and fight infections again.
Approximately 30 minutes before the transplant, you will be given medication to ease any side effects during the infusion. You will be closely monitored, and your vital signs will be measured frequently during and after your infusion.
Stem cell infusion
If the stem cells come from a donor who is not your blood relative, they are delivered directly to MSK. Here they are processed for transplantation into an unmodified or T-cell-free transplant.
Transplantation of non-T-cell stem cells
In a T-cell-free stem cell transplant, the T cells are removed from the stem cells before being transplanted into you. If you have a T-cell-free stem cell transplant, the procedure will be performed after the T cells have been extracted in the laboratory. It will take 12 to 24 hours, so the transplant will occur the next day after the donor provides you with stem cells. The stem cells will be injected directly into your tunneled catheter using a large syringe. There are usually no side effects from an infusion of T-cell-free stem cells.
Cord blood stem cell transplant
The cord blood will be thawed in our laboratory and brought to your room in a unique bag on the day of transplantation. If you are also receiving a transplant of partially compatible (haploidentical) T-cell-free stem cells from a relative, they will be given the same day you receive your cord blood infusion or the next day. The cord blood will be injected into your body through a tunneled catheter. Sometimes patients experience nausea or high blood pressure during a cord blood transfusion, and such phenomena will be eliminated as they occur.
