Continuity of Care

Continuity of care is the idea that patients have an ongoing relationship with their doctor. This ideal is associated with increased patient satisfaction, better adherence to treatment advice, lower hospital admissions and reduced use of medical services.

However, in the context of a healthcare system that is increasingly complex and fragmented, it is becoming more difficult to achieve continuity. This is because health professionals, who often specialize in one area of medicine or another, have to work within a multi-disciplinary team to provide care for each patient.

In addition, it may not be easy for a doctor to establish a solid rapport with a patient and their family. If a practitioner does not build trust with their patients, they will be less likely to retain those patients, which will impact on the availability of continuity of care.

This study aimed to explore how professionals involved in the chain of care for people with complex and multiple conditions across healthcare and social care systems in three different regions in Sweden perceive and value continuity of care (CoC) and to identify factors that promote CoC. A qualitative design with individual, paired and focus group interviews was used. The interviewees were selected to include a wide range of care providers and professionals with as much diverse experience of the aim under study as possible.

The interviewees discussed the importance of long-term and person-centred relationships, dynamic stability in organizational structures and joint responsibility for cohesive care provision across the different levels of the system, as factors promoting continuity. They also highlighted that better opening hours throughout the year were necessary to increase accessibility and a sense of security for patients, while individualized and written information on where to call and whom to contact with questions were essential.

Studies that compared the receipt of continuity of doctor care with measured mortality are necessary to answer this question, but they are rare and have not been well investigated. These are observational studies that control for confounding factors and have a design to limit the impact of reverse causality.

Virtual Nurse Triage

Virtual nurse triage is a convenient, affordable solution that allows patients to get care when they need it the most. By calling a triage nurse, patients can receive safe, accurate medical advice that can help them determine whether they need to seek emergency care or go home. This reduces unnecessary trips to the emergency room and saves parents time and money while increasing patient satisfaction.

Nurses who provide telephone triage have experience assessing and diagnosing patients’ medical conditions. They also have excellent communication skills and the ability to interact effectively with patients on the phone. They must be licensed in the state they work from and have a high level of knowledge about medical terminology, disease processes, and treatment options.

Hospitals have embraced virtual nurse triage as an efficient and cost-effective way to provide patient care. The process helps prevent unnecessary ER visits and urgent care appointments while reducing overall health care costs by 2.5 to 3.5 times.

In addition, virtual nurse triage can improve the patient experience by providing more frequent and accurate communication between the provider and the patient’s family. The system can be used to support a wide range of care needs, from admissions and discharges to chronic disease management and remote patient monitoring.

Telehealth technology is increasingly woven into the fabric of hospitals across the country, as well as in pop-up clinics and home care settings (Hollander & Carr, 2020). The most successful telehealth programs are ones that combine cutting-edge software with experienced human providers.

Nurses who implement telehealth technologies are responsible for training and educating their patients and families on how to use them (CMS, 2020). They can provide access to resources such as videoconferencing platforms, remote patient monitoring tools, and electronic health records.

They can also monitor the progress of the patient’s condition by analyzing patient data, providing feedback on symptoms and recommending treatments or referrals to providers. They can also help patients with their care plan, ensuring that patients are able to adhere to their personal goals.

In addition to delivering the best possible patient care, virtual nurses are important partners in transforming healthcare delivery systems into high-performance organizations. They can also help organizations build a strong telehealth workforce.
