Continuity of Care
Continuity of care is the idea that patients have an ongoing relationship with their doctor. This ideal is associated with increased patient satisfaction, better adherence to treatment advice, lower hospital admissions and reduced use of medical services.
However, in the context of a healthcare system that is increasingly complex and fragmented, it is becoming more difficult to achieve continuity. This is because health professionals, who often specialize in one area of medicine or another, have to work within a multi-disciplinary team to provide care for each patient.

This study aimed to explore how professionals involved in the chain of care for people with complex and multiple conditions across healthcare and social care systems in three different regions in Sweden perceive and value continuity of care (CoC) and to identify factors that promote CoC. A qualitative design with individual, paired and focus group interviews was used. The interviewees were selected to include a wide range of care providers and professionals with as much diverse experience of the aim under study as possible.
The interviewees discussed the importance of long-term and person-centred relationships, dynamic stability in organizational structures and joint responsibility for cohesive care provision across the different levels of the system, as factors promoting continuity. They also highlighted that better opening hours throughout the year were necessary to increase accessibility and a sense of security for patients, while individualized and written information on where to call and whom to contact with questions were essential.
Studies that compared the receipt of continuity of doctor care with measured mortality are necessary to answer this question, but they are rare and have not been well investigated. These are observational studies that control for confounding factors and have a design to limit the impact of reverse causality.
