Sunglasses with Strap
A lot of people remember to bring their sunglasses, but forget a retainer strap that attaches to the sunglasses and wraps around their neck. In the event your sunglasses go flying on a class IV rapid, that strap will keep them around your neck and not in the river.
A Long-Sleeve Sun Hoodie

While you might have various pieces of UV-protective clothing (shirts, shorts, pants), consider a lightweight long-sleeve hoodie specifically.
