For too long, people with chronic headaches and nerve compression syndromes have been dismissed with the phrase “it’s all in your head.” But invisible pain is real - even when it doesn’t show up on standard scans. Many patients endure relentless discomfort only to be told that their MRI is “normal” or that stress must be the cause. This not only delays treatment but undermines patient confidence and trust in medicine.

The problem isn’t that the pain doesn’t exist - it’s that the diagnosis often misses its true source. Peripheral nerve compression involves small superficial nerves - like the greater occipital or supraorbital nerves - being irritated or trapped by tight muscles, fascia or anatomical variations. These structures are too subtle for conventional imaging, but they can send constant distress signals that feel sharp, radiating or migraine-like.

When physicians rely on scans, they risk overlooking what the patient’s body is trying to communicate. Validation begins with listening - acknowledging that pain is a signal, not a flaw. Clinical history, physical examination and tools like diagnostic nerve blocks can reveal patterns that imaging might miss.

Too many people with chronic pain are left in “diagnostic limbo,” labeled with vague conditions and treated with skepticism. But absence of radiographic evidence is not evidence of absence. True care means expanding diagnostic frameworks, embracing mechanism-based thinking and validating the lived experience of patients.
