Most gynecology practices lose a large amount of revenue due to billing errors. In-house billing teams often find it difficult to navigate the complex nature of billing, especially the coding part. Gynecology billing involves a lot of diagnostic procedures, surgeries, and treatment codes. A single mistake in the coding procedure can cause your practice to experience claim denials, payment delays and revenue leakage. Therefore, your coding team needs to have a sound idea about different CPT codes associated with gynecology coding.

Current Procedural Terminology (CPT) codes for gynecology are divided into several main categories: Evaluation and Management (E/M) codes, surgery codes, radiology codes, and laboratory/pathology codes. These codes cover a wide range of gynecological services, including diagnostic procedures, surgeries, and treatments. Here are some CPT codes in each category that you need to use often in your gynecology billing process.

Frequently used CPT codes used in the Gynecology billing process:

 Evaluation and Management (E/M) Codes

You should only use E/M codes to bill for office visits and outpatient services, specifically related to women\'s reproductive health in gynecology.

  • -99202-99215: Office visits ranging from brief to comprehensive, considering patient history and decision-making complexity.
  • 99384-99387: Annual wellness visits for new patients (ages 18-39). It includes comprehensive evaluations and preventive services.
  • 99394-99397: Annual wellness visits for established patients (ages 18-39)
  • 99401-99404: Preventive counseling and risk factor reduction, such as contraceptive or STI counseling.
  • -81025: Urine pregnancy test.
  • -88164-88167: Cervical cancer screenings (e.g., Pap smear).

Surgery Codes

Your coding team uses the surgery codes for documenting gynecological procedures. The coding team needs to categorize these codes by the type of surgery and approach (vaginal or abdominal).

  • 58150: Total abdominal hysterectomy (removal of the uterus and cervix).
  • 58570: Laparoscopic total hysterectomy.
  • 58956: Unilateral oophorectomy (removal of one ovary).
  • 58700: Salpingectomy (removal of one or both fallopian tubes).
  • 58353: Endometrial ablation (destruction of the uterine lining).
  • 58140: Myomectomy (removal of uterine fibroids).
  • 7421: Cervical biopsy.

Radiology Codes

You should use radiology codes to document the imaging services related to gynecology only. These codes are categorized by the type of imaging and body area.

  • 76830: Transvaginal ultrasound of the female pelvis.
  • 74740: Hysterosalpingography (X-ray of the uterus and fallopian tubes with contrast dye).
  • 72195: MRI scan of the pelvis.
  • 72193: CT scan of the pelvis.

Laboratory/Pathology Codes

These codes are used for lab tests and pathology studies in gynecology.

  • 88141, 88142, 88143: Pap smears for detecting abnormal cervical or vaginal cells.
  • 87624, 87625: HPV testing.
  • 87491, 87591, 87850, 87800: Testing for STIs such as chlamydia and gonorrhea.
  • 76856: Pelvic ultrasound.
  • 58100, 58110, 58120: Endometrial biopsy.
  • 58555, 58558: Hysteroscopy (diagnostic or surgical examination of the uterus).

Also, note that the American Medical Association (AMA) updates the CPT codes annually, therefore, the categories and codes may change over time. Unfortunately, most gynecology practices often lack a team of professional gynecology coders to handle the entire coding job. This is the reason why; gynecology practices often outsource their coding and billing responsibilities to third-party gynecology billing and coding companies. Outsourcing partners allow gynecologists to fully focus on patient care as they care for the entire.
